Conference Scholarship for PT Students
As the oldest manual therapy fellowship in the country, the
Kaiser Hayward PT Fellowship is dedicated to enriching the learning
experience for future practitioners and is pleased to announce our
new Conference Scholarship for PT Students. The conference
scholarship covers the student’s conference registration and
provides $500 for travel, lodging and meal expenses.
We’re now accepting applications for a scholarship for the American
Physical Therapy Association Combined Sections Meeting being held
February 9-12, 2011 in New Orleans. Applicants must be students at
the time of the conference.
Applications are due by November 1, 2010. Mail or e-mail completed scholarship application form and essay to:

Mentorship & Fellowship Scholarship Program
Bilingual Scholarship
We are pleased to announce a 50% tuition reduction, for the Mentorship or Fellowship, to an individual who is bilingual in English and either Chinese, Spanish, Vietnamese, or East Indian dialects. One recipient per year.

A limited number of scholarships from Kaiser Permanente and the alumni organization (Institute for the Advancement of Orthopedic Physical Therapy) are available.
Funding Distribution
Scholarship funding is awarded based on a combination of academic merit and financial need and is to be used toward tuition and fees for the program.
Eligibility for Scholarships
Candidates must:
- Be a current applicant to the Fellowship or Mentorship programs.
- Have demonstrated financial need.
- Submit all required application materials with program application within the established deadline.
Recipient Selection
- Meeting all eligibility criteria does not guaranty that an applicant will receive funding.
- Recipients will be notified within 6 weeks of the application deadline.
Scholarship applications must be submitted with your application to the program. Candidates do not have to be Kaiser Permanente employees to be eligible to apply.
How to Apply
- To be considered for a scholarship, the application must be postmarked by the deadline date for admission to the admission to the program and included with your application to the program.
- Submit the following application materials:
- Scholarship application form
- Copy of pages 1-2 of your most recent Federal Tax Form 1040. If you were not required to submit a form 1040, please include an explanation in your personal statement.
- A 1/2 to 1-page personal statement describing why you are applying for financial assistance. Please include information about the following, if applicable:
- Are you currently paying off student loans? If yes, what monthly amount?
- Will you have any other sources of income during the program (e.g. parents, working spouse, your own part-time employment)?
- Are you the caretaker for any individuals other than those family members listed in your tax return? Describe.
